      The classical appearance of hypertensive heart disease – When assessing hypertensive heart disease, focus on:
      πŸ’‘ Morphology of hypertrophy: Typically, a rather symmetric thickening of the left ventricular walls (lateral wall, septum, and posterior wall), though some asymmetry may occur.
      πŸ’‘ Secondary findings: High blood pressure often leads to sclerosis of the aortic valve and mitral annular calcification.
      πŸ’‘ Variations: Some patients may present with a septal bulge or a so-called sigmoid septum, which is a localized thickening of the basal septum, commonly seen in elderly patients and associated with aging and long-standing hypertension. While usually a benign finding, it can occasionally cause left ventricular outflow obstruction, especially if the basal septum is significantly thickened or the aortoseptal angle is decreased.
