LV apical aneurysms are dangerous because they: 

🔍 Form areas of scar and fibrosis → perfect substrate for ventricular tachyarrhythmias. 

🔍 Create electrical instability with re-entry circuits → higher SCD risk. 

🔍 Indicate advanced disease and adverse remodeling in HCM.

🔍 Carry additional thromboembolic risk due to mural thrombi.
